Well, timing depends on a few things. First, whether you are using milestone* or experience, DM should tell you (*Not called Milestones in the book/DMG).
With milestones, you level up when the DM decides. Experience you level up based on the experience chart. DDB tracks levels based on experience for you (you just have to manually add the experience).
When it is time to level up, just click on edit character or "manage character and levels" depending on what page you are viewing. Click the level of the class you want to increase or add another class. Then just scroll through and fill any highlighted blue features.
Well - if you run a published module, it will tell you when to level up. I think.

Mechanically, just as DxJxC mentioned. Thay access their character sheet, push the level up button and add the new stuff as it comes up. On pen a paper, just refer to the class level you gained in the book and add the new stuff, gaining new HP maximun on the way.
Practically and thematically, as a DM, you decide when and how that happens:
- Automatically like a video game where just by resting or finishing a particular event or story point the players gain the new abilities (reaching a milestone).
I picture this way to be like the characters have always been researching, training, practicing or just the fact of "gaining experience" during their travels and adventures, and they just unlock the knowledge/features at that time because of it (as they gain a level).
- Or maybe the players need a small time off, a couple days, a week, a month, a year depending on your campaing, to train and gain the knowledge of the new abilities (indicated as they reach a milestone or the XP reaches the next threshold). This way, level up happens only on downtimes between adventures/events so the players can unlock new abilities for the next part of the quest (in 3.5ed there was a rule for level up that worked like this and you had to spend some time and money to find trainers/materials/knowledge or whatever to gain the level up benefits).
