Both are entirely up to the DM as neither is defined in the rules.
I would say though that it's worth keeping in mind that attacks, damage, hit-points etc. are all abstractions so just because you dealt damage doesn't necessarily mean that there's a wound as such, as depending upon what exactly is happening HP could be a measure of wearing down your enemy's defences so you can get past them and deal a killing blow.
In actual sword fighting for example you're not just standing there hacking each other down like trees until one of you wins (even though the game mechanics can make it feel that way); in actual sword fights the bulk of your time is spent dodging and parrying, and when you finally do land a proper hit that's usually the fight over. There's freedom in how to narrate it of course, like maybe you're scoring minor hits (cuts and scrapes, damage to armour etc.) so that you're "bleeding" the enemy and wearing them down until you can land the killing blow.
For weapons stuck in the body, the only thing I could think of in RAW would be to leverage the optional Disarm rule in the DMG; maybe after being hit by a solid attack you take the Disarm special attack on your turn and narrate it as the weapon still being in the wound when you grab it? By default a disarmed creature drops the weapon, so to stay with RAW you'd have the enemy pull the weapon part way out as you struggle for it, so it gets dropped as normal, otherwise you'd need to ask your DM to let you keep the weapon.
As for pulling a weapon out of a wound, that'd be pure homebrew; I definitely don't think it counts as drawing a weapon in the normal sense since it's not a weapon that you've stored "ready for use", and even a generous DM should probably throw some improvised damage onto what is certainly going to hurt!

Yeah entirely up to DM in a sense a character can thrust and pull back a sword from a body freely as part of the attack and unless you're is trying multiple item interactions on your turn, pulling a weapon from wound should easily be done in tandem with your movement and action.

Does pulling out a weapon from your wound count as "drawing weapon"?
Also, is it ok if players want to stab their weapons into enemies' body and leave the weapons there?
