Mike Mearls created a couple of homebrew monsters that I found. The Wrack Spawn has the following trait: " When the wrack spawn takes damage that does not reduce it to 0 hit points, every creature within 10 feet of it, that does not share this trait, takes 1d6 psychic damage." Does that mean it happens after each attack if a PC has multiattack?

I'd say yes. Until it hits 0hp.
