In the doorway literally in the frame and on the other side of the door would be different to me. The doorway being it's own space. Doorways are bottlenecks and choke points to me...again may be differ between DMs.
Hostile creature in the door no you cant pass, on the other side yes but essentially only one creature can be in a doorway at a single time. If you were to open a door inward with a creature on the other side that's an athletics check to force that door open and drive the hostile back.
As far as diagonal attacks go if a creature is in the doorway yes, but beyond the door no.
I had this situation in my last session. I just gave the target of a sideways strike across a doorway a +2 AC for cover. I only allow someone to move through an enemy square as difficult terrain with an acrobatics check, but in a tight spot I would do it with disadvantage.

If an enemy is directly on the other side of an open door, am I allowed diagonal movement through the door?
Are allies on either side of me able to attack the monster in front of me, diagonally through the door?
