Can a rogue always hide for an enemy? In plain sight? Behind a friendly companion? With a +12 on hide checks, he almost ALWAYS has the extra 3d6 for stealth attack...

I think we play it wrong, but I could be mistaken.
Sneak Attack doesn't require you to be Hidden. But no, if there is nothing to Hide behind, you would just be automatically seen if you try and Hide. Halflings can use their allies as cover and thus can Hide behind allies, but otherwise you need something reasonable to break line of sight and be able to Hide.
Sneak Attack can also proc if an ally is adjacent to the target and the rogue doesn't have disadvantage, and... rogues are built around always having that 3d6 damage (or more or less depending on rogue level) because they don't get Extra Attack like Fighters do. so if a rogue doesn't always have Sneak Attack going, either they are a really bad combat rogue, or they or the DM don't understand how Sneak Attack works.

I would not say the rogue always has sneak attack but it is certainly usually. If the party are trying to stay away from a creature with very strong melee attacks then part of the cost of that is the rogue might not get sneak attack unless he can successfully hide. If the rogue rolls high on initiative, which is quite likely given his dex, he also might not have a target adjacent to an ally.
Hiding for advantage does require something to hide in/behind Which may or may not be available.
Ask your DM about objects, size and shape, also the terrain... try and determine if it is possible to find something to hide behind.
The rules for hiding say the DM determines if it is possible to hide... a quote from the PHB. hiding rules 'You can't hide from a creature that can see you clearly, '
There is feat called [feat]Skulker[/feat] that allows you to attempt to hide in lightly obscured conditions. (Don't have to break line of sight.)
Most times it is a collaboration between DM and player, the DM letting the player know of it was possible and the player be creative and trying to find a way to hide.
PS: About the sneak attack damage, a good rogue will have many ways to choose from to get sneak attack damage every round.
Remember the rules say an enemy of your target next to your target, not an ally... so many get it dead wrong and say ally.
