Say a player wants to play a sidekick as has been suggested in several places. Do they have to start as an NPC or creature then add the sidekick levels? Or can they "build from scratch": rolling stats, picking race, etc? If the latter, what hit die do they use? Is it solely based on size like it is for monsters and NPC's?
On a semirelated note, I assume a level one sidekick starts with the hit points in its base statblock, and gains its first additional hit die at second level?
the sidekick rules assume you start with a creature statblock...I don’t see any issue with rolling stats, etc for a sidekick PC main (I wouldn’t allow it for a true sidekick secondary), and for hit die I’d find a suitable analogue from the creature list and use its hit die, or base it on the creatures size (d6 for small, d8 for medium, d10 for large)

There are no rules for building a player character through the usual process but using a sidekick class instead of the normal classes. I mean, it's pretty straightforward if that's something you want to do, but the sidekick rules assume you're going to start with a monster stat block. If the goal is to give the player a really simple character then having them go through the normal process kind of defeats the point. At that point you might as well just play one of the four subclasses from the Basic Rules.
Sidekicks gain one hit die every time they gain a level. That includes first level.
