I am currently reading the Ravenloft novel "Knight of the Black Rose" by James Lowder which tells the tale of how the Death Knight, Lord Soth, becomes trapped in Barovia and butts heads with the vampire Strahd von Zarovich.
I have not finished the novel yet, so please no spoilers for the ending.
There was one part of the story that I found to be really interesting and something that could be put into a Curse of Strahd campaign.
This is the portal that the Vistani hero Kulcheck discovered (and the one discovered by Soth with the aid of his vistani guide). When Soth and Magda and Azeal try to access the portal, they have to fight the Guardian, which seems to be this creature with a multitude of eyes and mouths and which uses visions to disarm each of the characters.
What I want to figure out is what would be a good monster in 5e to match the Guardian. My first thought was a Gibbering Mouther, but the monster presented in the 5e MM seems far less powerful than the one illustrated in the story. I was wondering if anyone else who had read the story might have suggestions for what monster the Guardian could be.

I haven't read it but based on your description and how Dread domains work in general maybe something like Greater Starspawn Emmissary I think captures the adversary you're trying to project.

Maybe, but this seemed like just a large abberation that is driven by hunger or something. Star Spawn Emissarys seem like they have some sort of "greater purpose" to their presence. Also, a greater star spawn emissary is probably beyond the CR of Curse of Strahd, but I was considering trying to raise the CR so that could work

I’d just find the stat block for when everyone has escaped the visions and is ready to get down to rolling dice. Then, write the visions you want and just glue them together.

Fair, there's a whole spectrum of Star Spawn that might fit whatever CR you settle at.
