The worm on the string dangles from its short line in a cheerful burst of color and googly eyes. So long as the user holding the worm holds the artifact by the string and does not touch the worm, no effect occurs. If a user touches the worm, the artifact comes alive. At the start of the user's turn, the user takes 1d8 piercing damage as the worm begins to burrow through the skin. If the user or another creature applies fire or successfully drags the worm out by its string (DC 25 Athletics) before the end of the user's next turn, the worm on a string returns to its inert state.
Otherwise, the worm will disappear from view and begin to force its way through the meat and bone of the victim. At this point, the user may elect to cut out or amputate the body part holding the worm if applicable, at a DC 20 Medicine to succeed. For every round the worm remains unstopped, the user suffers 1d10 piercing damage. After four rounds unstopped, the worm reaches the victim's chest. The victim suffers 10d10 damage. Roll a d20. The resulting number is the number of newly birthed worms on strings which force their way out every orifice on the victim's body and return to their inert state.
