This melon-sized green decanter is shaped like a warty green toad, and is stoppered by a stringed cork.  It is full of a sickly-sweet odored, mucous-like substance. 

The fortune teller from Neverwinter divined that this potion may only be identified by the previous owner(s), by someone with greater skill than her [Arcana DC 30], the Identification spell cast with a 5th level spell slot or higher, or by a Wish spell.
