This clever device can transform from an ordinary pocket watch into a spider-like automaton designed to assist with emergency triage. Incredibly, the "tic doc" is capable of suturing small wounds with surgical precision in less than 6 seconds.
The tic doc is a Tiny construct with the stats of a spider with no attack and proficiency in Medicine. If the construct drops to 0 HP, it is broken and cannot be used again until it is repaired by someone proficient with tinker's tools.
The tic doc can be activated and thrown up to 30 feet as a bonus action. Once it is activated, the construct enters initiative. On its turn, it moves toward the nearest ally most in need of healing, and can hitch a ride when necessary. Once per turn, it stitches up the wounds of a creature it is touching, restoring 1d4 hit points.
The construct functions as a combat medic and triages patients as reasonably as possible. It deactivates whenever combat ends and refuses to assist except in emergency situations. The wind-up construct cannot be activated more than once per short rest.
