Weapon (greataxe), artifact (requires attunement)

You have a +3 bonus to attack and damage rolls made with this magic weapon. It has the following additional properties.

Tolrash's Mind. You gain proficiency in history while attuned to this weapon.

Sentience. Tolrash's Anger is a sentient chaotic neutral weapon with an Intelligence of 18, a Wisdom of 18, and a Charisma of 13. It has hearing and normal vision out to a range of 60 feet.

The weapon can speak, read, and understand Common and Orcish, and can communicate with its wielder telepathically. Its voice is high pitched and reedy. While you are attuned to it, Tolrash's Anger also understands every language you know.

Tolrash's Curse. Being born naturally ugly and generally reviled by the people around him, Tolrash places on a curse on the soul who is wielding this weapon. While you are attuned to the artifact, your body rots over the course of four days, after which the rotting stops. You lose your hair by the end of day 1; finger tips and toe tips by the end of day 2; lips and nose by the end of day 3; and ears by the end of day 4. A regenerate spell restores lost body parts.

Personality. Tolrash's Anger speaks with an imperious tone, as he enjoys the sudden possibility of power.

The sword’s purpose is to destroy anything in its path as an act of vengeance. It doesn’t care who lives or dies, including the wielder. The axe believes that it must repay all beings for the pain it endured during life.

Proficiency with a Greataxe allows you to add your proficiency bonus to the attack roll for any attack you make with it.

This weapon has the following mastery property. To use this property, you must have a feature that lets you use it.

Cleave. If you hit a creature with a melee attack roll using this weapon, you can make a melee attack roll with the weapon against a second creature within 5 feet of the first that is also within your reach. On a hit, the second creature takes the weapon’s damage, but don’t add your ability modifier to that damage unless that modifier is negative. You can make this extra attack only once per turn.
