Potion, legendary

This vial of blood sloshes around within its jar even when not moving, and howls of ravenous hunger can be heard from within. The black blood seems to have a mind of its own as it seems to be smashing against the glass on its own accord. There is a tag around the bottle that reads, "Sip only, don't drink". The pure amount of demonic energy in the blood is too much for a regular mortal body to hold so it must be sipped. When sipped, several things happen. If you are smaller than large, you grow to become large, your strength, constitution, and dexterity increase by 6, your hp increases by 100, you take on an appearance of Yeenoghu, you gain a bite attack that deals 3d6 piercing damage you can use a bonus action to heal yourself half of the damage you did, your hands sharpen into claws making your unarmed attacks deal 2d8 slashing damage, you can attack three times per turn, can dash as a bonus action, and you gain a resistance to all damage except force and psychic. However, there are a few drawbacks. If you do not damage a creature during your turn, you take 10 necrotic damage that you do not resist, if you do not attack a creature during your turn, your next turn is spent moving to the closest creature and attacking them as many times as possible, and you cannot cast spells while transformed. The transformation lasts for 1 minute and once the transformation is over, your stats and hp return to the way they were before the transformation. The the bottle that holds the demon's blood has enough for 5 sips. If you have a good alignment and sip the potion you take 2d6 necrotic damage when you transform back. If you drink the bottle instead of sipping it, you take 8d8 necrotic damage, your alignment becomes chaotic evil, you crave for violence, you have an insatiable craving for flesh, and your alignment becomes chaotic evil.
