The token can be attached to any piece of clothing.  While it's on your person, you gain a +2 bonus to saving throw DCs of your blood curses.  (To do this, you will have to manually change it in avrae.  By doing -dc #)

Additionally, you increase your hemocraft die by the next die size, lvl 1-4 is d6, 5-10 is d8, 11-16 is 1d10, and finally 17+ is 1d12 (Have to manually change this)
