World Codex Talisman (Dormant)
- Mind’s Clarity. You gain a +1 bonus to Dexterity, Intelligence, and Charisma. You also gain Proficiency in History, Religion, and Survival.
- Echoes of the Past. You can recall any memory you have personally experienced with perfect clarity, including minute details like scents, sounds, and emotions.
- Unveiled Truth. You always know when a lie is being told that you can hear being spoken. Anyone who lies to you can now be magically tracked with accuracy, always aware of their physical position, out to a range of 100 feet for 1 hour. You can only track a single target at a time.
- Arcane Archive. Once per Long Rest, you may store a single memory within the talisman, which can later be recalled in full detail by you or shared telepathically with another willing creature. The memory can be up to 1 hour long.

Mnemonic Ward. You have Resistance to effects that alter, erase, or implant memories, such as Modify Memory or Geas.

Idealized Potential. Your maximum Hit Points increase by 30.

Historic Lore. You can cast Speak With Dead up to 3 times per day before needing to complete a Short Rest to use this ability again.

Whispered Secrets. You gain Advantage on Perception and Investigation checks made to detect traps, hidden doors, or concealed objects. You can also cast Detect Magic at will.

World Tome. You can manifest a large book as a Bonus Action, revealing a detailed map of the region you are in. It records towns, notable places, businesses, and other features. The book also automatically records your life, choices, and events around you in detail, including written words and images. You can unsummon the book as a Bonus Action.
World Codex Talisman (Awakened)
- Mind’s Expansion. Your bonus to Dexterity, Intelligence, and Charisma increases to +2. You also gain Expertise in History, Religion, and Survival.
- Truthbinder. You can cast Zone of Truth at will within a 60-foot range. If a creature attempts to lie within this effect, they take 30 points of unavoidable Psychic damage and their falsehood is stopped.
- Unveiled Truth. You can now magically track those who lie to you with improved accuracy, always aware of their physical position, out to a range of 200 feet for 8 hours. You can track a number of individuals in this way equal to your Proficiency Bonus.
- Arcane Archive. The talisman can now store up to three memories, which can be replayed at will. You can project these memories instantaneously into the minds of up to three creatures within 60 feet, allowing them to experience the event as if they were present.

Mnemonic Ward. You gain Immunity to effects that alter your memories or compel you to act against your will, such as being Charmed.

Idealized Potential. Your maximum Hit Points increase by 60. Moreover, you regain 5d10 Hip Points back every minute.

Historic Lore. You can cast Speak With Dead up to 9 times per day before needing a Short Rest. You can also ask an additional question each time you use this feature.

Whispered Secrets. You can sense hidden chambers, passageways, or mechanisms within 30 feet, even if they are magically concealed. Once per Long Rest, you can unveil the history of a location or object you touch, revealing significant past events or previous owners.

World Tome. The book now provides a map of the entire continent you are on, including neighboring regions, kingdoms, and empires. It also reveals notable people, animals, and monsters as moving icons. The book can be given to an ally, and by spending 1 hour, they can imprint their life record into the book as a separate chapter. Any ally who does this instantly shares their recorded history with you as if it were your own.
World Codex Talisman (Exalted)
- Mind’s Perfection. Your bonus to Dexterity, Intelligence, and Charisma increases to +3. You also gain Advantage on History, Religion, and Survival skill checks.
- Truthbinder. You can cast Zone of Truth at will within a 120-foot range. If a creature attempts to lie within this effect, they take 50 points of unavoidable Psychic damage and their falsehood is stopped.
- Unveiled Truth. You can now track those who lie to you with supreme accuracy, always aware of their physical position, out to a range of 500 feet for 24 hours. You can track a number of individuals in this way equal to twice your Proficiency Bonus.
- Arcane Archive. The talisman can store an unlimited number of memories. You may share a stored memory with a number of creatures equal to your Proficiency Bonus and can display them as a semi-physical illusion in a 30-foot radius. You can also pull information from the World Tome to enhance these memories.

Absolute Truth. The talisman now has 3 Charges. You can cast Modify Memory by expending 1 Charge. You regain all expended Charges after completing a Long Rest. Additionally, once per Long Rest, you may force a creature within 60 feet to relive a memory (real or fabricated) so vividly that it becomes their new reality for up to 1 minute. During this time, they are incapacitated unless they succeed on an Intelligence and Charisma Saving Throw.

Idealized Potential. Your maximum Hit Points increase by 100. You now regain 1d10 hit points at the start of your turn (every 6 seconds). Moreover, if you have lost a limb, it can be regrown in 8 hours.

World Tome. The book now reveals a map of the entire world. You can project a 100-foot by 10-foot holographic globe from the book within 15 feet of you as a Bonus Action. It also tracks known and unknown dungeons and ruins, though unexplored areas remain obscured. You can select a single Waypoint on the map, creating a visible golden path leading you there. The path may not be the safest but will be the fastest. Only you and a number of allies equal to twice your Proficiency Bonus can see this golden trail. You can choose to end this effect for any ally as a Bonus Action.
