PAM if you want to do more damage. But, maybe you’re getting by just fine with GWM’s bonus action from killing things (because that’s realistic of a lvl 4 barb with gwm and 18 str, especially if reckless attack is used frequently, which it totally should be), in which case you could consider other feats to round out your character.
you could take res dex if your deck score is odd, then you Improve your initiative and AC and your dex saves, which has synergy with danger sense.
you could take alert, granted it would be a little redundant with Feral instinct. But alert with advantage on initiative would be pretty sweet.
you could take mobile, just to ensure you reach your targets.
you could also take skill expert to make you more versatile. Expertise in stealth is great on an unarmored barb. Maybe you’d rather have expertise in athletics, make grapple checks even easier.
I have a sick magic great sword. As for feats only phb is available. I was going between resilient wisdom and mobile. Mobile seems nice because if I can kill the spellcaster before he casts a spell that targets my wisdom save.
Polearm Master and Great Weapon Master do exceptional damage if the fight is melee oriented. I played a half orc champion fighter/bear totem barbarian with PAM & GWM. The barbarian's advantage on the attack rolls, helps offset the -5 penalty to hit. Even better if a party member can cast Bless on the party, then you're really set to do damage!
What is your con score? Assuming you’re unarmored, you could get hp and ac. And your dex; again ac but also initiave and a pack of skills. If they’re both odd numbers, I’d take a long look at rounding both up.
I am a level four bear barbarian with gwm and a 18 str do I take a feat or increase my str. If a feat which one.

I really enjoy Sentinal. Most rounds you can end up with a reaction attack.
At lvl 4 we need to know what your dex and con are.
18 str is good, but if dex and con are both 15, your better off doing 1ea on those for ac, hp, dex ac, and saves.
if dex and con are 18, then pick one of the 3 to 20 😆
built a lvl 20 version of your self, add items from your dream list and play around with asi and feats so you have a plan.
Max out your strength and constitution scores but keep it in mind that I find the tough and slasher feats really helpful for a barbarian.
