If I buy the players handbook, do I have EVERYTHING? Or do I have to also buy those additional options for say I want all the backgrounds and traits and stuff? Currently have the handbook and subclasses and spells in my cart. Should I buy them all or just the handbook?
All the character options, monsters, etc. in the PHB are included in the $29.99 purchase. The individual listings are for people who don't want to buy the whole book, but just want to buy individual pieces. (for example, I bought just the Tabaxi from Volo's)

If you buy the whole PHB, you'll unlock the character options and spells. You do not need to buy those separately.
Purchasing any of the Source or Adventure books on DDB unlocks all options within that book for use, but only the options within purchased material.
So EVERYTHING on the additional is obtained buying the $29.99 handbook?
