I'm on version 2.1.1, use DnD Beyond digital dice is turned on, token is selected prior to clicking initiative on DnD Beyond, and I wait for the roll to finish before navigating away from the tab. The roll completes on DnD Beyond but nothing appears on Roll20.
Can you press F12 to open the dev console on both the roll20 page and dndbeyond page, go to the Console tab and clear its content, then make a roll. See if an error shows up, and then send me that information if that's what you get. I can't think of anything that would prevent it from rolling other than a bug I haven't seen before.

And, as far as I know, there is no way to integrate the Beyond app on the phone with Roll20. This could change with the public API which is on Beyond Roadmap. (I've seen it on their Trello.) But I guess it would take sometime.
The current best way of integration is Beyond20 (thanks to KakarotoDM). But you must use both Beyond and Roll20 on your browser to make it work.

If it has not been already asked, are you working on a similar extension for Fantasy Grounds by any chance? I know very little about programming and a DM friend of mine wants to run a game on Fantasy Grounds but I am not liking the idea of not using DnDBeyond...especially now that you made this amazing extension.

That's really weird, because from the log, it doesn't look like you're rolling anything at all, and even says that the sidepanel is set to '_default" (not initiative) which is the sidepanel when you first open the page.
I assume you have the quick rolls option enabled and do you see the Beyond20 icon when you hover on initiative? what happens if you disable the digital dice in your character sheet ?

Although the roll came across to the roll20 chat it did not add me to the turn tracker. I'm fairly certain however on my home machine I do NOT have quick roll enabled. Could that be the cause?

I think I got what's wrong. This happens sometimes when I am playing with my group. In order to add the roll to the Roll20 Turn Tracker, the token must be selected on Roll20 (just click on the token before changing to the DDB tab and clicking Initiative).
But as players sometimes (I mean, everytime) forget to click on token before rolling init, I just add the token manually (right click, add to turn) and type the initiative they've already rolled.

Yes, quick rolls has to be enabled for what you're trying to do. Otherwise the only way to roll with Beyond20 is to click the "Beyond20" icon from the side panel, which you can't open if you have digital dice enabled unless you click just outside the digital dice box but still within the initiative larger box, and it can be tricky.
Check what other options you may have changed from the default, there is one about whether or not to add the initiative to the tracker. The token for whom the initiative is to be added also needs to be selected as MaxBraga mentioned above.
First, I want to say that I LOVE this addon! It's fantastic and I can't wait to use it.
I have a couple questions/suggestions.
- Is there going to be a way for DMs to get information from all characters in a DnDBeyond campaign inside of roll20? (Group passive perception, group passive insight, DM based secret group skill checks, languages, etc).
- Are you going to stick to the "other templates" format or do you plan to add support for the community contributed 5e character sheet?

If you just use Roll20 and DnDBeyond there is no interaction between the two. You'll have to have a character sheet in Roll20 to do all of your rolls. You can use DnDBeyond to keep track of everything but at minimum you'd have to load your Roll20 sheet with your stats, weapons, and spells.
With Beyond20 you just have to make a token in Roll20 and it'll link everything from DnDBeyond to it essentially. So any rolls you make on DnDBeyond will kick over to Roll20 automatically. It's a fantastic tool and I couldn't recommend it more highly.

It’s essentially a bridge between D&D Beyond and Roll20. When you’re using Beyond for your character sheets with all the management happening on this site, Roll20 has no way of essentially making use of it. Your sheets, rolls, calculations and many other things need to be done manually on Roll20 with their built in systems. But with beyond20, you can integrate both, making it much easier for players and the DM by letting D&D Beyond handle all the heavy lifting and allowing Roll20 to take that data through Beyond20. It’s a plug-in that saves time and keeps the game running without making the DM or players pull their hair out.
If anyone is wondering, it works in the current version of Edge as well since Edge is built on top of Chromium and allows Chrome Extensions to be installed within Edge. It looks tiny bit different but still works just as well.

Fantasy Grounds is a separate app, right? Would a chrome extension be able to plug in?
I Cancelled my Master Tier Subscription January 12th 2023 because of "OGL" 1.1 - Resubscribed 28th of Jan, now the SRD is in CC-BY-4.0
Fantasy Grounds is a standalone application, not a web based app. Since it doesn't run within chrome itself, there is no way to make it work with it.
Also, you should convert your DM to using Foundry and ideally on The Forge (forge-vtt.com), since that's just the superior VTT :)
Disclaimer: I'm a huge fan of Foundry VTT and use it myself, and I'm the owner of The Forge (hosting service for Foundry).

Yeah I had the token selected as I used to mess that up all the time. Turns out I did NOT have the quick roll enabled and that was the cause.
