Need a list of plot twist please. I only want an idea cause most stories have them. I don't think that all d&d stories need them but it's always good to keep the players on their toes.
Rather than approach this looking to put in a plot twist, consider your world and the factions competing within it. How might a villainous character or group try to fool the players and others to gain the upper hand or conceal their plans? The answer might be a plot twist.
It doesn't have to be earth shattering. An early quest I created had the party investigating a village after contact had been lost. Wolves had recently become active in the area and were suspected of making the roads too dangerous to travel. As it turned out, a bunch of bandits had taken over and were taking advantage of the wolves to cover what they were doing.
i do have an evil lich faction, a group who follow a lich master who was defeat oh so long ago. Maybe the lich guys are doing something under the guises of the dragons actions. Theres also a faction of Orcus cultist, their a bunch of murder hobos.
The lich faction is looking for their own thing which would bring back their lich master. What could i do with that?

But without knowing what type of story/adventure you are writing/creating it'll be hard ot give you much more than that.
