would this ability, in theory, let two star spawn hulks deal infinite damage to each creature within 10 ft of them? The one is dealt psychic damage, and it is reflected in a 10 ft radius. This would activate the other’s psychic mirror, and the process repeats.
Is this a bug? It feels like any self-respecting cult of Tharizdun would realize this, and be absurdly powerful.

As written, yes, there's a potential for infinite damage. Most DMs should be wise enough to rule that Psychic Mirror cannot trigger itself, though the text doesn't say this.
interesting concept, a wise DM would keep each star-spawn hulk 15 ft away from each other but a malicious DM who actually wants to kill his players would not.
an idea would be to make it so the psychic damage repeat, repeats only a certain amount of times for each Star-spawn hulk like it repeats 1d6 X the amount of star-spawn hulks are there.
I discovered this the day I got Mordenkainen's Tome of Foes. I was reading into all the weird monsters and aliens, and discovered this hack, and vowed to use it in at least one of my campaigns.
I haven't used it yet, but I will use it sometime.
Keep in mind, this will destroy your players, instantly kill them and any other creature not immune to psychic damage with 10 feet of either Star Spawn Hulk. (does this apply to bacteria and small insects? If so, this is the best antibacterial spray ever.)
If your players aren't immune to psychic damage, make sure there is a relatively easy way for them to end the battle and revive their fallen party members. Otherwise, TPK!

I’m considering using it as a puzzle, where they can’t get too close to the hulks.
Keep in mind, it is instant death. Not only will knock your character to 0 hit points, but will cause 3 failed death saving throws immediately.
