My DM gifted me with an item similar to a weapon in the video game Monster Hunter
Weapon: A sword and shield combo (acts as longsword), when it crits, a charge is gained and stored (up to 5 charges max) and can be converted to a greataxe with Reach. While in greataxe mode, 1-5 may be expended at a time to add d4/charge to the attack dmg.
Are there any mode convertable weapons I could use as a base?
Unfortunately not, and currently it also isn't possible to add actions to a magic item either, which would be the ideal way to handle this.
I think your best bet is to create this as a magic longsword with 5 charges, specifying in the description what you can do with it, then on your character sheet just add a greataxe and set its weight to 0, so you're only taking the weight of the longsword. Equip both, use the longsword to track your charges, and the greataxe as reference for the greataxe rolls.
You'll need to roll the added damage manually based upon how many charges you spend, but I don't think there's another way to handle that; you could add custom actions doing various numbers of d4 in slashing damage for convenience, but it's only really a minor improvement over just using the roller (or rolling the physical dice if you're playing in person).
Sounds like a fun weapon idea though! Could combine really well with a Champion (Fighter sub-class) if you're playing as one.
Former D&D Beyond Customer of six years: With the axing of piecemeal purchasing, lack of meaningful development, and toxic moderation the site isn't worth paying for anymore. I remain a free user only until my groups are done migrating from DDB, and if necessary D&D, after which I'm done. There are better systems owned by better companies out there.
