Freyalis cackles out and hits the frog with a skeletal hand that clings to it's back causing 3DMG
The frog just wants it's meal and with Tormir in it's mouth it jumps 20' into the pond. Landing with a splash you see Tormir disappear underwater as the frog swims another 10'. You can still see the frogs back above the water but the dwarf is submerged.
Tormir? How long can you hold your breath for?
Suffocating. A creature can hold its breath for a number of minutes equal to 1 + its Constitution modifier (minimum of 30 seconds). When a creature runs out of breath or is choking, it can survive for a number of rounds equal to its Constitution modifier (minimum of 1 round).

Jim follows as closely as he can without going into the pond to a depth he cannot wade. Given that he can no longer see Tormir he sees no danger in hurling a ranged attack. He casts Fire Bolt at the creature.
That seems to happen sometimes. No idea why. Just roll them again. In fact, I've seen people edit the post to roll again in the same post, and doing that has sometimes made the first rolls work.
(Tormir: I was kind of surprised about how long someone can hold their breath but it makes sense. Without the modifier it's 1 min. As to attacks, yes, you have disadvantage underwater)
Jim wades out into the water a couple feet before he feels his feet starting to get sucked into the muddy silt on the bttom of the pond and stops and cast firebolt onto the retreating frog. It hits it's back causing another 5 Dmg.
Even in the water you can see the frogs skin blister and it's looking pretty messed up.

Tormir considers breaking the grapple, but is just too full of rage (I'd lose my rage if I don't attack, and all I would be able to do this round is break the grapple anyway, so like a good barbarian, I'm just going to hack away!).
Good point and it pays off. After being burned the frog momentarily loosens its jaws enough for you to shift around to get a good angle of attack. Turning more into your side you're able to swing your axe through the water. Even though it seems like it's moving slowly the axe buries itself in the frogs eye KILLING it. You're able to get yourself out of its mouth as it's jaws go limp and swim to the surface.
We are out of initiative for the moment. However, upon Tormir reaching the surface you see something baking a beeline towards him and the dead frog just under the surface of the pond. It's coming from the other side of the pond is around 20' from Tormir, 50' from the rest of the party.
(Does tormir see it coming? If so...) Tormir grabs the corpse of the giant frog, and pushes it as hard as he can in the direction of the disturbance in the water, as he attempts to swim back to land.
Yes, Tormir, you can see the ripples in the water moving towards you. You push the frog in that direction and start swimming away from it. You get about 5' away from the frog when a crocodiles jaws come up out of the water snatching the frog and then taking down under the water while rolling and thrashing. You continue your swim and make it back to shore safely seeing a large cloud of blood in the water where the crocodile was.
As he calms, and the red mist behind his vision clears, Tormir says, "I agree about not wanting to become a snack, but I'll be damned if we're going to go all the way back and round this swamp again. I say we need to keep going, even through the water, until we reach that higher ground. Plus, what's the worst that could happen? That was just a flesh wound!"
Lorros picks Tim's body back and everyone looks wearily at the water. Tormir you are correct that you can't keep completely out of the water to continue on a direct route. You'd have to backtrack and find another way around. The fight with the frog wasn't exactly quiet though and you don't see anything else in the vicinity. It's likely that if anything else was in the area it would have either joined in already or been scared off.
So you all keep continuing on. Trying to stay out of the water as much as possible. After being attacked everyone is on alert and hearing regular frogs start croaking again makes you guys jumpy but you don't encounter any other danger. Finally reaching higher ground the landscape changes from standing dead swamp trees to an old growth forest. It rises up a slight hill and you find yourselves on a small road. Looking to the left (North) the road extends straight for as long as you can see. Looking to the South it curves slowly to the left and disappears around the bend about 1/4 mile from where you are. You don't see anyone else on the road.
