Could it be cast right under a monster and the other portal cast onto the side of a cliff or underside of a bridge or suspended walkway ejecting the monster to it's doom? Or at the very least be used to move a monster safely away?
Provided you can see the flat surface where you create the portals, as DM my only caveat would be wether creating a portal in a creature's space is the same as the creature entering the portal it. For example, spells affecting creature when their area of effect appears are specifics rule, in general it's not the case for all spells and the Hither-Thither Staff doesn't say when the portal appears, each creature in it exits from the other, but when they enter.
Unless you can float or fly realistically if a portal opens underneath you you're going fall through it. That's how portal guns in scifi work, no reason a portal staff would be different.

Even when its not your turn some degree of movement is assumed, your dex bonus to your Ac, reflex saves all assuming you have some movement happening in your 5 foot square. So it would not matter when you cast the exit or entrance hole. If I were the DM and a player asked this I'd make it a DC 10 dex/acrobatics check for a medium creature, and a DC 15 for a small creature. Making the check would mean you stepped to the edge and did not fall in at all, failing by less than 5 you fell through and caught a ledge,(basically a prone effect) and you'd need to fail by 5 to fall through. And generally no if you ended the spell with them 1/2 in it they would not be cut in half, there are a lot of effects like where teleports end up in the wrong space etc and the general rule is they get shoved into an unoccupied space though sometimes they take some force damage in like phasing effects where they end their movement in a object.

Yeah that all makes sense but still going back to portal guns I've never seen someone not fall through a portal that appears under them. I'd only give saving throws to creatures known to have fast reflexes and able to think quickly. You open a portal underneath the average person they're probably falling through. I know I would. No way would I be able to react in time to grab the ledge. If it's a dimwitted and/or slow monster being sent through the portal I'm not giving them a saving throw. And if they do grab the ledge just close the portal and cut them in half.

No. These types of effects only deal damage if they say the do. In general, a creature is pushed harmlessly out of the portal (see Forcecage). In this case, the creature would be forced out to the safe side (The simplest solution would be either to say it is pushed out to the side of its choice or it is pushed back out the way it came).
Secondly, a successful save or missed attack does not indicate barely clinging on, it means that it did not catch the target in the portal and the creature did not enter it at all.
