I have a homebrew world named Reyic, which I have 2 groups playing on the continent of Audenia. I've gone for an early renaissance kind of vibe, with one group being effected by firearms swinging the outcome of an empires quest to unify the continent and the second grappling withe the early effects of colonisation.
I have a world called Aetherius. Its a high magic world (In spots, one continent in particular, Lauda is a mageocracy). But dragons are dying off after Bahamut and Tiamat killed each other, leading some of the more intelligent of the species to seek desperate measures.
Currently, the party playing is trying to stop Dendar from breaking free from the Shadowfell, the yuan-ti are making it difficult however.
My homebrew world is like the Forgotten Realms, where there are multiple different places. One is the main continent of Kobold's Axe. The capital is Ithia, which is sort of a cross between Waterdeep, Baldurs Gate, and Sigil (Modrons and Slaads are competing for influence over the city.) Far off from this continent is Devil's Claw, a dystopian realm not unlike Dark Sun ruled by elemental warlocks, giants, dragons, goblinoids, and vampires. Finally, there's a third continent I've been working on called Cloud's Eye with Dwarven Sailors and Egyptian-style elves.
I have a homebrew world called Vale, which is a flat water world with just one small continent. It is basicly a demiplane that is accessible by sailing straight into the worst storms. Eventually you come out the other side to Vale. It's a highly magical world with no gods, though the gods do exist their; they just don't worship them. Instead, they believe in animism, and the mystical order of the Velen (a secret sect), who maintain balance between the worlds. In recent years there has been a civil war between the Barbarian Hordes of the Fjordvale (a northern city), their allies the Bloody Fangs (a vampire cult) vs. the witches of Witchvale and the Velen.

My current homebrew world is Avaigne. It blends themes from Arthurian myths, English folklore, and German fairytales. There are three main adventurers' guilds the players had to choose between: the Order Errant (questing knights and scholars), the Huntsmen of Saint Perrault (Witchers if the Brothers Grimm wrote them), and the Merry Company (basically Robin Hood knockoffs).
