I know that a lot of people say that Earth Tremor is a bad pick for a 1st level slot. My impression is that this is because the difficult terrain created by the spell makes it more difficult for the caster in the center of the effect to leave the area.
Aside from using a spell like Misty Step or having the Athlete feat or Monk levels to jump away, what are some ways to minimize the effect of leaving difficult terrain when there are two or more enemies within that terrain and within 10 feet of you? Or is this spell designed to be wonky to use on purpose?
You don't use Earth Tremor to escape enemies; that's what spells like Expeditious Retreat are for. The best way to use Earth Tremor is to knock enemies prone for other melee combatants to deal with, or make it harder for enemies to escape, run past you, or reach someone else. It's a good Eldritch Knight spell.
I have a Tiefling character who is Forge Cler 2 / Sorcer X with this spell. I figure that between my +1 chain mail, shield, Shield spell and Mirror Image, I can make this spell worth something without taking too much damage.

My feelings exactly. My Warforged eldritch knight got some good mileage out of this spell when surrounded by skeletons.
Using an earth genasei you are affected by the difficult terrain and can freely move. I took this spell using the druidic initiate feat for my monk and its great for making a barrier between a crowd and your casters while you skate around the terrain.

I think you meant earth genasi are not affected by the difficult terrain. Yes, I agree with you. There are a lot of ways to deal with difficult terrain in this game and being Earth Genasi or going to level 6 in Land Druid are some of them.
