My players gazed into a cursed mirror in a haunted house and had their bodies possed by ghosts (who were trapped in the mirror). The player's souls were trapped within the cursed mirror which is a flipped reflection of the house. Now they need to escape the mirror realm and get their bodies back.
Looking for any ideas for combat encounters, puzzles or loot they might find!
It's none of your requests, but you could have them occasionally see what the ghosts are doing with their bodies. Brownie points if it's something they wished they never seen.

I don't normally endorse this sort of thing, but if you want a sort of metagamey way to make players feel like they're in a topsy-turvy world, consider throwing challenges that require them to roll low numbers on the d20.
Also, if the players use AoE attacks, especially those that deal thunder damage, have their surroundings crack and shatter, creating difficult terrain that deals slashing damage.
I've had an idea for a magic item that you could homebrew too. I don't have the details down yet, but it would allow the character to treat their line-of-sight as though it started in another nearby space. The narrative is that it reflects/refracts their vision such that they appear to see from a different location. It would act sort of like an echo knight's echoes.
Another idea: have a scene with lots of funhouse-style mirrors that distort one feature of the PCs. If the PCs touch one of the mirrors, they are temporarily distorted as if by the mirror. You could consider giving them special abilities or penalties as a result.

as the characters move through the funhouse style maze, touching surfaces may lead to one of the party being replaced and trapped in the mirror realm while the doppelganger is now free to terrorize the rest of the group as he sees fit.
Could just be one single to really amp up the tension on "who's it gunna be?" or a number of them for a little more an action packed feel.

I think the fractine and mirror shade are perfect for this.
