I've always wanted to play a character who was a werewolf but despised the thought this character would only be CE...essentially killing everything it could get its hands on. I know it's a great role playing hook for a neutral or good character to struggle with this disease and the consequences of the murder they commit. But I also like the the thought of Were (creature who are almost all evil) that are neutral or goodly aligned. I still think being ferocious and blood thirsty can be part of CN or N or even CG good characters make up.
Anyone played werewolves / rats/ boars / in this capacity in their games?
Chaotic evil does not automatically mean murder hobo. It means you don't follow laws, don't honor agreements, etc and are actively harmful (not necessarily aggressive) toward people. You can be chaotic evil without killing a single person.
Anything is possible through homebrew (rules or setting). The MM is only referring to forgotten realms cannon.
In forgotten realms the weretiger is neutral and werebear is good. So being bloodthirsty can be part of those alignments.
For official options, I recommend shifter.
