Last session is pulled a bit of story out of my ass. Specifically a sickness that has befallen the Town. My idea was that a monster that lives in the fountains of the town.
But i have not thought of the monster that actually causes this sickness and now i am looking for said monster. My first idea was a hag but i am unsure as to what kind of hag AND if maybe there is a better monster which is even described as living in the sewers and spreading disease.
My party consists of 4 level 3 PC's (Rouge, Paladin, Cleric & Sorcerer) and the disease was described as starting in the arms end legs with a numbness. Spreading up and changing the skincolor to grey as it progresses.
If you want to go hag pick which one you think would work best for you and you can just re-skin them to suit your needs. You could even go with one of the water humanoids and re-skin them where they can cause this disease. You could even go that it's a poison that is resistant to the cure disease and cure poison spells, and you can create a quest to find the antidote.

Remember that it's a game and the goal is to have fun. So let's go kill some monsters.
Fomorian, some kind of weird Stench Kow, a Gibbering Mouther, maybe a Two-Headed Troll, or Oozes. Those always stand out in my mind when I think of creatures that just breed disease. Reskinning them to fit the theme will be necessary, but thematically imo they work pretty decently.
Perhaps there is an old forgotten sewer system connecting the fountains, if there isn't one already. You could definitely do a hag taking up residence there, with a swarm of rats, as well as a couple oozes that she's poisoned. Perhaps she was once a witch the town went to for occult-y needs before someone tried to rally against her and she fled, and secretly found her way back into town via the sewers.
This could give you a dungeon crawl experience - which the PCs will have to ask around about to find - and surely she's trapped the place up nice and tidy. Then maybe if they try to kill her she pleads with them to spare her, she'll antidote the town if the town agrees to take her back in, she just wants to return home (maybe someone suggests investigating an "old abandoned cottage" that used to be her home and finds out about her history with the town through journal pages, maybe a map to the sewer entrance in there and some prototype poisons/antidotes). The PCs could choose to have some RP with her and the townsfolk to try and talk things out, or kill her, or maybe find her an alternate place to live. Lots of options with something like this! Maybe she didn't even mean to poison the town with her experiments but she's not been able to synthesize a full antidote yet, and can send the PCs on a good ol ingredients fetch quest to help her.
A Night Hag (cr5) has Plane Shift x2/day, so maybe during the day she uses that to hide away and returns at night to her experiments.
Maybe reskin a Water Weird to have a poison ability. Or a sea hag?

If you wanted to go homebrew, the first thing that comes to mind is some kind of Corrupted Water Elemental. Kinda like the Golgothan, but water-based. And so maybe it lives in sewers and swamps and the brackish water of the abandoned swimming pools of old haunted mansions. So you'd start with the stat block of a water elemental (CR 5), but anyone hit by it needs to make a CON save (DC 12) or get infected with the sickness and thus suffer a -1 penalty to all attack rolls, skill checks, and saving throws until they complete a long rest (or get cured by Greater Restoration). And that penalty is cumulative with each failed save!
Or maybe the same kind of thing, but based on an Air Elemental. I mean, after all, "malaria" is literally the Latin for "bad air". Google "Miasma Theory" for more in depth history of the origins of that if you're interested.

Black Dragon. Several of their Lair-related abilities are related to poisoning water specifically. Give the B.Dragon a spell or item that allows for casting of Polymorph and the dragon can easily hide in the town itself.

The monster doesn't have to be one that the party is supposed to kill in this campaign. An Adult bracket B.Dragon could become a recurring villain. Just make sure you have some back-up plans to ensure there is a good excuse for the Dragon to not just kill them as soon as they find out.

Successfully completed the Tomb of Horrors module (as part of playing Tomb of Annihilation) with no party deaths!

I picture an otyugh or diseased rats as being able to defile a water source and spread disease.

Maybe the hag has a pet. :P You can always change stats a bit to be more appropriate to your group regardless of the existing statblock.

My players did this a couple times. Had a whole antlion cave system. They stealthed through it. Twice.
Sorry I'm late but I think almost any ooze monsters could do the trick or add to the sewer dungeon if they have not done it.

A Wyrmling is the least likely age bracket of Dragon to have access to Polymorph, but the DM should choose what/whoever best suits their narrative.

The monster doesn't have to be one that the party is supposed to kill in this campaign. An Adult bracket B.Dragon could become a recurring villain. Just make sure you have some back-up plans to ensure there is a good excuse for the Dragon to not just kill them as soon as they find out.
