If you have an activated Ioun Stone of Leadership for the Charisma bonus, what tactics can you use to keep enemies from targeting the stone? (Yes, i'm aware of its stats.)
Another creature must use an action to grasp or net the stone to separate it from you, either by making a successful attack roll against AC 24 or a successful DC 24 Dexterity (Acrobatics) check. Ways to prevent it include staying out of reach, making it unable to take actions or otherwise giving it disadvantage to such check.
Anything that makes it more difficult for the enemy to target you also works for the stone. Plaguescarred covered them.
I would also add to only use it when you need it. If you are walking around town having the ioun stone circling your head might tempt an opportunistic thief.
Having said that how likely it is to happen does depend on the game. On may tables having the DM give the player the joy of finding a powerful magic item only to take it away at the next encounter would be considered a jerk move and one that is not fun for the table. In my gaves I have never had an Ioun stone targetted.
Something I am not sure about is whether it can be attacked by weopons other than a net. It has an AC and HP so in theory I think it can but the Ioun stone says "another creature must use an action to grasp or net the stone to separate it from you, either by making a successful attack roll against AC 24 or a successful DC 24 Dexterity (Acrobatics) check." grasping and netting does no damage and MUST implies this is the only way of seperating it from you. That is one to discuss with your DM

The rules for Making An Attack don't excludes objects worn or carried;
Choose a target. Pick a target within your attack's range: a creature, an object, or a location.
While not official ruling, the Dev also an attack will tell if it excludes object worn or carried on Twitter;
@armando_doval When the rules for making an attack or casting a spell talk about targeting an object, is it implied that it's an unattended object, or it possible to target someone's armor/shield directly with an attack in an attempt to damage/break it?
@JeremyECrawford If a game effect lets you target an object, the text of that effect tells you if worn/carried objects are prohibited. The rules don't assume that "object" means "object not currently worn or carried by anyone."

There's already an option for an opponent to take it out of play for at least the duration of the encounter, and in general destroying a player's stone is likely to generate bad feeling at a table.

It depends on the DM but I would allow it. There are at least 2 uncertainties.
"You make yourself--including your clothing, armor, weapons, and other belongings on your person--look different until the spell ends or until you use your action to dismiss it."
An Ioun stone is considerered to be an item you are wearing I would say that means it is "on your person" but another DM might disagree
Does each individual item look different or does the whole lot look different. If each item looks different then you would not be able to appear unarmed (a common use of disguise self in my experiance) and while you might be able to make a helmet appear like a baseball cap you could'nt appear bare headed as you can only change the appearance of your helmet.

could you give it a costume to look like a familiar? Gazer maybe?
