An Ioun stone is named after Ioun, a god of knowledge and prophecy revered on some worlds. Many types of Ioun stone exist, each type a distinct combination of shape and color.
When you use an action to toss one of these stones into the air, the stone orbits your head at a distance of 1d3 feet and confers a benefit to you. Thereafter, another creature must use an action to grasp or net the stone to separate it from you, either by making a successful attack roll against AC 24 or a successful DC 24 Dexterity (Acrobatics) check. You can use an action to seize and stow the stone, ending its effect.
A stone has AC 24, 10 hit points, and resistance to all damage. It is considered to be an object that is being worn while it orbits your head.
Fortitude. Your Constitution score increases by 2, to a maximum of 20, while this pink rhomboid orbits your head.

This seems... just simply worse than they Manual that gives you a +2 bonus to a stat, possibly worse than an amulet of health. The rarity of this item just feels off, as it pales in comparison, especially as it requires your attunement.

It is generally worse than the Manual, but consider that not all items of the same rarity need to be of the same power level.
Also, there are certain situations where the Ioun Stone is better. You might not have the time needed to study the Manual, you might want to change who gets the con bonus in a couple levels, or perhaps you're Larloch and want to commit to an aesthetic.

The major difference with the Manual is that the Stone is reusable (within your lifetime). The Manual, once read, loses his property and regains them in 100 years. The stone can be passed to whoever needs it the most in the party, as long as you have an attunement slot free. It is better than the Amulet of Health if the starting CON is 18+, same as the amulet with CON 16 (18 or 19 CON gives you the same +4).
Does attuning to this Ioun increase your hit points while it's equipped, and is it retroactive?

Except you can use it in addition to a manual of bodily health.
