This is an old, decrepit scroll that notes the steps necessary to provide a new chance for any one creature. Hailing from a time before such spells as True Ressurection and Wish had become the new standard among mages, True Rebirth has long been banned for its barbaric requirements and the crimes it tended to spawn--few still remember it and even fewer consider it a viable option despite it being a cheaper alternative to the other spells and having the advantage of not requiring any spellcasting to use, instead needing only the connection to a divine being and the steps described below. Which being, if any, would still answer the call of this ancient ritual is anyone's guess...
It can turn a construct into flesh and blood, bring a vampire back from the clutches of their curse, provide a new vessel for a disembodied soul, and much much more; be warned, however, for this ritual is neither pleasant nor an easy feat to achieve. Not only will you have to wholeheartedly and without regret accept the transformation, but you must also die.
There are different parts to this ritual that must all be completed in order for the process to succeed, deviance from any one of these could potentially result in unknown disasters.
You must find and kill a heifer three years old, a female goat three years old, a ram three years old, a turtledove, and a young pigeon. You must then split them in half, with the exception of the fowl, and lay the pieces in two rows, leaving a path through the center just wide enough to walk through. It is important the carcasses are fresh and free from disease for the Covenant to take effect.
Two large emeralds, worth a total of at least 500 gp, must be placed in the eye sockets prior to death in order to preserve the soul.
You must procure amniotic fluid, as fresh as is possible, which you must consume immediately before walking the path of the Covenant; to allow for the rebirth.
To complete the ritual you must first prepare the animals.
Then imbibe the amniotic fluid and walk through the path whilst chanting the following words in order to seal the Covenant:
O Divins, audi votim meum.
Renati sit anima mea.
Emundatur ab omnibus quod est invitum.
Ostende mihi negatus misericordiam.
Da mihi formam quam cupio.
Renascatur.
At the end you must kneel with this scroll in front of you, and commit the ultimate sin; whether this is instantly fatal, caused by yourself or someone else, as well as how your eyes are replaced by the emeralds is irrelevant; all that matters is you must die, your blood must touch this scroll, and before your soul leaves your body the emeralds must be in place.
Once this is all finished, and if the ritual has been successful, your body will fully decompose within 3 days. One hour after the sound of the emeralds falling into the skull is heard, they will have combined into one which can then be removed.
The emerald will darken in color in a process that will take place over a number of days depending on how gruesome the condition/s which need/s to be healed. On the dawn of the last day, the emerald will crumble, and you shall return to the land of the living in the state which you have desired.
Should you have performed the ritual on your own the emerald will remain in the empty skull, this will not affect the results in any way, you will simply return wherever the emerald remains.
