A gold ring, with the band made to resemble several interlocking long bones, with an oversized ivory skull, inlaid with two red gems in the eye sockets.
As you place the ring on your finger you have a vision, you see yourself commanding a skeleton.
When you complete a killing blow against an opponent, the flesh falls off the skeleton, and then it rises, taking the equipment its former self used. It functions as a regular undead skeleton, no external magic capabilities, just what holds it together.
You do not have the ability to control any skeleton other than one/s you have killed.
If your skeleton/trophy takes damage so do you, the skeleton cannot be healed, you can. The skeleton regains it health once you have had a long rest. Should the skeleton get to 0 hit points its bones turn to dust, you cannot heal for 1 round of combat.
Should the skeleton get further than 500 ft away from you, it falls to a pile of dust, and you permanently lose 1 hp.
Should you wish to remove the ring you need to roll a wisdom save with a DC25, if you remove the ring you need roll on the indefinite madness chart.
