Partial Incorporealness. The Visage is not technically incorporeal, it cannot pass through solid objects, but it can move through very small openings, like keyholes.
Multiattack. The Visage can attack twice with its claws.
Claw. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 13 (2d8 + 4) slashing damage plus 7 (2d6) psychic damage.
Assume Identity. The visage takes on the physical form of any creature that it has killed. This disguise works as if the visage had cast Disguise Self.
Ludicity Control. The visage can control the perceptions of others. As a bonus action, the visage can control the perceptions of up to six targets. It can change an entire terrain, make objects seem further or closer away, create subtle changes on building design or architecture, change what the target hears, smells, sees, feels, and more. It can even completely cut off all of the target's senses.
There is no save associated with this if the target does not suspect the illusions. If the target believes their senses are being manipulated, they can make a Wisdom Saving Throw DC 15 to see past the illusions for one round. This effect lasts as long as the Visage maintains concentration. (Constructs and Undead are unaffected by this effect.)
Description
Out in the Planes, the rising and falling of Powers is a fact of life. New deities come into existence from the silvery Astral Plane, see their mortal followings grow, and eventually fade away, their corpses entering a millenia-long sleep adrift in the Astral.
Sometimes, and only sometimes, one of these Powers will awaken from that long sleep, wrested from its reverie for whatever reason. This new god-infant finds itself thrust into a world it knows nothing about, but about which it has libraries of data. And once it has gained enough power, it finds the spirits of its former self's servants.
This is what a visage is, the risen spirit of a servitor of a dark god, newly returned to life. Such spirits have incredible power over the senses, being able to do anything from deleting an object from a victim's sight to completely altering the landscape around them. They are also capable of assuming the physical form of any creature they kill, making them very difficult to root out, and often instilling a healthy paranoia in their enemies.
