Gravitational Pull. Creatures within 120 ft. of the Void Elemental must make a DC 15 Strength Saving throw at the start of each of their turns or be pulled up to 15 ft. directly toward it. A creature who fails this save by 5 more more is instead pulled up to 45 ft.
Void Form. A creature or object who enters the Void Elemental's space takes 4 (1d4 + 2) force damage. If a creature or object drops to 0 hit points due to this damage, it is instantly destroyed. If a creature is exposed to this ability or the Void Tendril attack 10 times, they must make a DC 15 Constitution Saving Throw or become poisoned for 1 hour as they contract Radiation Poisoning.
While poisoned, the creature's hit point maximum is decreased by 5. Each subsequent failed saving throw against radiation while the creature is poisoned increases the duration the creature is poisoned by an additional hour and decreases its hit point maximum by an additional 5. If a creature’s hit point maximum is reduced to 0, it dies. Lastly, when a creature that has been poisoned due to radiation recovers from being poisoned, roll a d20. On a 1, the creature suffers a severe side effect from the poisoning: an aneurysm or a cancerous growth. The creature's hit point maximum is halved until it is healed by a greater restoration spell or similar magic.
Multiattack. The Void Elemental makes two attacks and can use their Void Stride ability if available.
Void Tendril. Melee Weapon Attack: +5 to hit, reach 10 ft., one target. Hit: 8 (1d6 + 5) force damage and 4 (1d4 + 2) radiant damage.
Radiation Beam. Ranged Weapon Attack: +4 to hit, range 60/180 ft., single target. Hit: 18 (4d6 + 4) radiant damage.
Void Stride (Recharge 4-6). The Void Elemental can spend a bonus action to briefly jump through the Void, traveling in a straight line to any unoccupied space it can see within 30 ft. Every creature and object that the Void Elemental passes through suffers from the Void Form ability.
Void Twist. When the Void Elemental is hit by a ranged weapon attack it can create a small rift in space to increase its AC by 2 against that attack. If the attack misses because of this increase the Void Elemental can choose a creature within 30 feet to become the new target for that attack. Use the original attack roll minus 2 to determine if the attack hits the new target.
Description
A terrifying creature formed from the essence of higher intelligence wrapped in pure void energy. This creature born of the darkest corner of the known planes knows nothing except consumption and destruction.
