Regeneration. Vladimir regains 10 hit points at the start of its turn. If Vladimir takes fire or radiant damage, this trait doesn’t function at the start of Vladimir next turn. Vladimir body is destroyed only if he starts his turn with 0 hit points and doesn’t regenerate.
Rejuvenation. When Vladimir's body is destroyed, its soul lingers. After 24 hours, the soul inhabits and animates another corpse on the same plane of existence and regains all its hit points. While the soul is bodiless, a wish spell can be used to force the soul to go to the afterlife and not return.
Special Equipment. Vladimir wields a Dragons Wrath Greatsword (Wakened) with a hilt sculpted to resemble silver dragon wings and a pommel shaped like a silver dragon’s head clutching a black opal between its teeth
Turn Immunity. Vladimir is immune to effects that turn undead.
Vengeful Awakening (Recharge after a Short or Long Rest). If Vladimir would be reduced to 0 hit points, his current hit point total instead resets to 190 hit points. Additionally, Vladimir can now use the options in the "Mythic Actions" section for 1 hour. Award a party an additional 3,900 XP (7,800 XP total) for defeating Vladimir after his Vengeful Awakening activates.
Vengeful Tracker. Vladimir knows the distance to and direction of any creature against which it seeks revenge, even if the creature and Vladimir are on different planes of existence. If the creature being tracked by Vladimir dies, Vladimir knows.
Multiattack. Vladimir makes two fist attacks or two with his +2 greatsword. If his Vengeful Awakening has activated he instead makes two attacks with his dragon's wrath greatsword (Wakened).
Fist. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 11 (2d6 + 4) bludgeoning damage. If the target is a creature against which the revenant has sworn vengeance, the target takes an extra 14 (4d6) bludgeoning damage. Instead of dealing damage, the revenant can grapple the target (escape DC 14) provided the target is Large or smaller.
+2 Greatsword. Melee Weapon Attack: +9 to hit, reach 5 ft., one target. Hit: 20 (4d6 + 6) slashing damage. Against Strahd, Vladimir deals an extra 14 (4d6) slashing damage with the weapon.
Dragon's Wrath Greatsword (Wakened). Melee Weapon Attack: +9 to hit, reach 5 ft., one target. Hit: 20 (4d6 + 6) slashing damage and 7(2d6) Cold damage. Against Strahd, Vladimir deals an extra 14 (4d6) slashing damage with the weapon. Vladimir can only use this attack if his Vengeful Awakening has activated within the past hour.
Ice Storm (Recharge 6). Vladimir unleashes a 30-foot cone of frost from his greatsword. Each creature in that area must make a DC 16 Dexterity saving throw, taking 8d6 cold damage on a failed save, or half as much damage on a successful one.
Vengeful Glare. The revenant targets one creature it can see within 30 feet of it and against which it has sworn vengeance. The target must make a DC 17 Wisdom saving throw. On a failure, the target is paralyzed until the revenant deals damage to it, or until the end of the revenant’s next turn. When the paralysis ends, the target is frightened of the revenant for 1 minute. The frightened target can repeat the saving throw at the end of each of its turns, with disadvantage if it can see the revenant, ending the frightened condition on itself on a success.
Vladimir can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature’s turn. Vladimir regains spent legendary actions at the start of his turn.
Move. Vladimir moves up to his speed
Devastating Blow. Vladimir makes an attack with a minus -5 penalty to hit, and he deals an extra 10 damage on a hit.
Sweeping Strike (Costs 2 Actions). Vladimir makes a large slash, choosing 3 squares within 5 feet of him that are adjacent to each other and makes an melee weapon attack against each creature in those squares.
If Vladimir's vengeful awakening has activated in the past hour he gains an additional 2 legendary actions and gains access to the actions below as well as legendary actions.
Frost Slam (Costs 2 Actions). Vladimir brings his sword in a massive overhand slam, causing icicles to sprout from the ground in a 30 foot line that is 5 feet wide originating from Vladimir. Each creature in the line must make a DC 17 Constitution saving throw. On a failed save a creature takes 18 (4d8) cold damage and is restrained by icy formations for 1 minute, or until it or another creature takes an action to break them free. On a successful save a creature takes half as much damage and is not restrained.
Pillars of Ice (Costs 2 Actions). Vladimir stabs his sword into the ground, causing sharp pillars of ice to stab upward in squares adjacent to him. Creatures in those squares must make a DC 17 Dexterity saving throw taking 22 (4d10) piercing damage and 11 (2d10) cold damage on a failed save or half as much on a successful one. Vladimir cannot take this action again until the beginning of his next turn.
Aerial Strike (Costs 3 Actions). Vladimir momentarily sprouts spectral wings and leaps into the air, temporarily gaining a flying speed of 30 feet. Anyone within 5 feet of Vladimir when he does this must succeed on a DC 21 Strength saving throw or take 15 (2d6+8) bludgeoning damage and be knocked prone. Vladimir then moves up to 30 feet through the air and lands in an unoccupied space. When he lands each creature within 5 feet of him must make a DC 17 Dexterity saving throw, taking 14 (4d6) piercing damage and 7 (2d6) cold damage on a failed save or half as much on a successful one.
His attacks should have an extra +1 to hit, due to his proficiency bonus being +4, not +3. Also, you should give him some legendary resistances.
