Vine Whip. Melee Weapon Attack: +4 to hit, reach 5 ft., one target. Hit: 6 (1d8 + 2) slashing damage. 

Poison Spray. Ranged Weapon Attack: DEX 13 to hit, range 15 ft., anyone within a 5ft radius of central target. Hit:  7 (2d4 + 2) poison damage, none on a success

Entanglement. If the plant gets hit by someone within 5 ft, the plant lashes out a vine which wraps around the person. The person being entangled needs to make a STR save (DC 13) or be restrained. If the person is still restrained on the plant's next turn, the plant will make thorns grow from the vine and the person being restrained will take 1d6 piercing damage for each consecutive turn they are entangled

Each vine shares the AC of the wraith itself, but only has 5 HP, so another party member could cut the entangled person free if need be. With each hit point the vine loses, the DC to break free from the entanglement decreases by 3
