Innate Spellcasting: The Voidweaver's spellcasting ability is Charisma (spell save DC 16, +8 to hit with spell attacks). It can innately cast the following spells, requiring no material components:
- At will: detect thoughts, minor illusion, darkness
- 3/day each: dimension door, greater invisibility, confusion
- 1/day each: feeblemind, dominate monster
Multiattack. The Voidweaver makes two Void Tendril attacks.
Void Tendril. Melee Weapon Attack: +9 to hit, reach 10 ft., one target. Hit: 15 (2d8 + 6) psychic damage. If the target is a creature, it must succeed on a DC 16 Intelligence saving throw or be stunned until the end of its next turn.
Reality Warp (Recharge 5-6). The Voidweaver creates a 20-foot radius sphere of distorted reality centered on a point it can see within 60 feet. Each creature in that area must succeed on a DC 16 Wisdom saving throw or be affected by one of the following effects (Voidweaver's choice):
- Temporal Distortion: The target's speed is halved, and it cannot take reactions until the end of its next turn.
- Spatial Twist: The target is teleported up to 20 feet to an unoccupied space the Voidweaver can see.
- Mind Fracture: The target takes 21 (6d6) psychic damage and has disadvantage on the next saving throw it makes before the end of its next turn.
Ethereal Shift. When the Voidweaver is hit by an attack, it can use its reaction to become ethereal until the end of the attack, causing the attack to miss. This ability can be used a limited number of times per combat (3 times).
The Voidweaver can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time, and it can only be used at the end of another creature’s turn. The Voidweaver regains spent legendary actions at the start of its turn.
- Detect. The Voidweaver makes a Wisdom (Perception) check.
- Tendril Swipe. The Voidweaver makes one Void Tendril attack.
- Illusory Shift (Costs 2 Actions). The Voidweaver projects a shadowy illusion of itself in a space within 30 feet until the end of its next turn. The illusion acts as though it is the Voidweaver, and any attacks against the Voidweaver have a 50% chance to hit the illusion instead.

Appearance: The Voidweaver is a shadowy aberration with a shifting, ethereal form adorned with glowing tendrils. It seems woven from shadows and starlight, with a constantly distorted appearance.
Nature and Behavior: This enigmatic creature hails from the edges of the cosmos, where reality is fluid. The Voidweaver is curious and capricious, often manipulating space, time, and the minds of those it encounters. It communicates telepathically, observing and influencing others with its intense gaze.
Environment: Voidweavers are found in places where reality is thin, such as ancient ruins or magical disturbances, often near portals to other dimensions.
