Unstable Form. The horror’s body constantly shifts and writhes. The first attack made against it each round has disadvantage.
Psychic Screech (Recharge 6). The horror lets out a shrill, mind-piercing shriek. Each creature within 10 feet must make a DC 11 Wisdom saving throw, taking 7 (2d6) psychic damage on a failure or half as much on a success.
Abyssal Tendrils. The horror’s writhing limbs lash out at reality itself. Any creature that starts its turn within 5 feet of the horror must make a DC 11 Dexterity saving throw or take 3 (1d6) necrotic damage as the tendrils lash their flesh.
Tentacle. Melee Weapon Attack: +3 to hit, reach 5 ft., one target.
Hit: 6 (1d8 + 1) bludgeoning damage, and the target must make a DC 11 Strength saving throw or be pulled 5 feet closer to the horror.

Vhorath – The Unshackled Horror
(Aberration of the Void, Bound by the Flan in Ages Past)
The Vhorath is an unnatural entity that should not exist in the ordered structure of reality. It is a being of warped flesh and flickering presence, seeming to phase between existence and nonexistence, as if it were never meant to be part of the Material Plane. Some scholars argue that the Vhorath originates from the Astral Plane, where it drifted for eons as a parasitic echo of forgotten gods. Others insist it is a thing of the Lower Planes, a creature born from the madness of the Abyss or the scheming cruelty of Carceri. Whatever its origins, it was deemed too dangerous to roam free and was imprisoned by the Flan long before the rise of modern civilizations.
Appearance
The Vhorath does not have a stable form. It is small and hunched, roughly three feet in height, but its body writhes unnaturally, distorting itself into unnatural angles. Its skin is pallid and semi-translucent, appearing like wet parchment stretched over a too-thin frame. The creature's head is featureless save for a vertical slit, which only opens when it emits its horrific Psychic Screech—a soundless wail that bypasses the ears and drills directly into the mind.
From its back and sides, twitching, blackened tendrils sprout like grasping fingers, constantly curling and unfurling as if searching for something unseen. These Abyssal Tendrils react to movement, lashing out at anything that comes near, and they seem to phase in and out of existence, as though the Vhorath itself is only partially in the Material Plane.
Behavior & Mindset
The Vhorath is not a beast, nor is it a demon or devil with clear goals. It is a mistake, a rip in reality, a thing that thirsts for thought and emotion, feeding off the fears of those around it. It does not understand language in the traditional sense, but it "listens" to thoughts, responding to strong emotions like pain, terror, and despair.
If allowed to remain in the Material Plane for too long, the Vhorath corrupts its surroundings. Shadows stretch unnaturally, whispers slither through the air, and the barrier between worlds weakens. Some Flan myths claim that when a Vhorath is fully unleashed, it becomes a beacon for greater horrors, calling forth others of its kind from the void between planes.
Sealing & Release
The ancient Flan, recognizing the existential threat of the Vhorath, used sacred glyphs and psychic barriers to trap it within stone prisons, deep beneath the earth. These prisons were never meant to be opened, only maintained. However, with the passage of time, knowledge of these bindings has faded, and now, as the world stirs and old powers rise, the seal on the Vhorath has begun to weaken.
If the gate is not closed in time, the Vhorath may fully emerge, and with it, something far worse might take notice.
