Void Fortitude. If damage reduces the bloom to 0 Hit Points, it makes a Constitution saving throw (DC 5 plus the damage taken) unless the damage is Radiant or from a Critical Hit.  On a successful save, the void bloom drops to 1 Hit Point instead.

Necrotic Spore Cloud. A spore cloud erupts around a single target within 15 feet of the Void Bloom. The target must make a DC 12 Constitution saving throw or be Poisoned until the end of their next turn.

Bonus Actions

Releasing Shriek.  When the Void Bloom becomes Bloodied (at half its HP or lower, 2024 rule), it emits a piercing shriek. All creatures within 15 feet must make a DC 13 Wisdom saving throw or be Frightened of the Void Bloom until the end of their next turn.

Description

From the corrupted earth at the heart of the Whispering Stones, the Void Bloom erupts with a sickening, wet sound. It is a grotesque mockery of life, a nightmare flower born of necrotic energy and ancient malice.

Its form is vaguely bulbous, resembling a monstrous, petaled plant, yet it moves with an unnatural, almost predatory grace. The central 'body' is a pulsating, dark purple mass, interwoven with thick, gnarled roots that seem to drink directly from the corrupted earth. From this core, several thick, thorny tendrils whip and writhe, tipped with jagged, blackened claws that crackle with dark energy. These aren't just roots; they are its limbs, capable of lashing out with surprising speed and force.

Where a flower would have petals, the Void Bloom instead unfurls layers of necrotic tissue, resembling dark, leathery leaves that shimmer with an oily sheen. These 'petals' are often torn or withered, covered in a fine, dark dust that seems to suck the light out of the air.

Its most unsettling feature, however, is its blight-covered 'head'. What might have once been a stamen or pistil is now a distorted, fleshy maw, perpetually agape, revealing row upon row of needle-like teeth. From this gaping orifice, a faint, chilling mist constantly billows, carrying with it the sickening sweet scent of decay and the faint, persistent echoes of the whispers that plague the Void from whence it came. When it moves, the mist trails behind it like a shroud, and you can almost hear the faint, disembodied whispers within the very air it exhales.

As it becomes Bloodied, the Void Bloom seems to draw even more intensely on the surrounding necrotic energy. Its pulsating core throbs more violently, and the thorny tendrils lash out with increased ferocity. A terrible, guttural shriek tears from its maw, a sound that feels less like a cry and more like the very fabric of reality being ripped apart, resonating with the ancient malice it embodies.

In its presence, the air feels cold and heavy, and a sense of profound unease settles over the area. It is a thing of unnatural growth and decay, a testament to the dark power that has festered beneath the ancient Whispering Stones.

Lair and Lair Actions

Whispering Surge: The whispers intensify, and up to two patches of difficult terrain (5ft squares) appear around the Void Bloom, as the ground becomes corrupted.

Grasping Roots: Entangling roots emerge from the ground. One creature within 20 feet of the Void Bloom must succeed on a DC 12 Strength saving throw or be Restrained until the end of their next turn.  While Restrained, they will have Disadvantage on all Attack rolls and a Movement of 0. They can repeat the save at the start of their turn as their Action.
