Personally, I love the variant versions of standard monsters that Ravenloft brought, such as different kinds of vampires resulting from the different races.
And in my own head cannon, Vecna is still trapped within the mists
From what I've come to understand reading up on Ravenloft, the entire setting is one made up of dark lords like Strahd. These dark lords divide the setting between them into baroque, antiquated fiefdoms, but are themselves prisoners of this world, by higher, darker powers. Some are native to Ravenloft, like Strahd. Others are taken in from other planes by the mists, like the player characters themselves.
Strahd, a native, wants to break free. Vecna, who had been captured by the mists, did.
That's likely a gross oversimplification, but I haven't dived all that deeply into it.
Conn, is partialy right, the Mists are powered by entities simply refered to as "The Dark Power", they scour the prime planes looking for choice souls often evil, though not always, they then snatch them away into the demi-plane of dread.
The Demi-plane itself can then either form a realm for that soul to rule over, but trapping them within a border that they cannot cross over, but they can sometimes influance it, the more vile the soul, now called a Dark Lord, often the more power they have over not only how their border reacts, but even the land itself may be effected.
Now not all souls pulled in may end up with a Domain as these realms are called, sometimes they are allowed to wonder fairly freely amongest the domains causing mischief and carnage as they go (like the infamous Gentleman Caller, who to date seems immune to the mists border closing powers of darklords), those such souls may end up gaining their own domain.
Now these beings, the Dark Powers are considered very powerful, for Vecna was a demigod when the mists snatched him up for their pleasure, they were not able to keep him imprisoned for long however, sometimes they even get bored of their subjects, like Lord Soth, and cast them out.
It's speculated that the Demi-plane may be a prison, or it may be a staging ground for worse to come.....
What is known, is the Barovia and Strahd were the first land and darklord, Azalin Rex the second Darklord, while the cultured town of Mordent with it's ghost Lord Godefroy became the 3rd Darklord and second domain, after the previous two attempted to escape there during a mysterious event (which the old 1980's module i10 covered).
Most of the Dark Lords come from other planes, a couple do indeed come from the mists, one of them is a Vistani of terrible power....
And the Vistani come and go to their hearts content unimpeded by the mists in most cases.....
Truly, I think they need to see where Ravnica takes them and afterwards conceivably accomplishing a greater amount of the MTG crossovers. We may see another old school like Ebberon (which I never played in as it assumed control for my most loved Greyhawk, so I naturally abhorred it), yet I am speculating with the current splatbooks, Spell Jammer as that will end up being the "Door" to different terrains.
Sadly I wouldn't lay the blame on Greyhawk being moved over in favour of Eberron during the shift to 3.5.
Greyhawk as been having legal issues for decades, after Gary Gygax got ousted from TSR there's been problems, as parts of Greyhawk such as certain characters and such have been suffering from multiple copyright issues, and since Gary's passing the mismanagement of his estate by his wife has pretty much killed off ANY new attempts to bring the setting back.
Fortuntely the older published stuff is becoming available, since WotC own those rights outright to those to my knowledge....
Possibly, but i also lay the blame at the shift from RPGA to (Dis)Organized Play. I didn't like that at all and felt that the shut down of the Triad and how things worked sucked. Even AL still has some of those issues now that they had back then that i didn't see with how the RPGA was run at the end.
