One problem with that... the tick has to be a Lyme disease carrier for it to work. So you turn into a tick and go get Lyme disease and then somehow get to your victim and bite them. Then you turn back and congrats, both you and your victim have Lyme disease .. oops! :P
(Plus restoration spells exist)
"Best" polymorph option is of course a highly subjective opinion. I stand by my recommendation despite all impracticalities consequences and down sides :P

What would you say is the best polymorph option? I would say giant ape, i've killed dragons with them.

Def Giant Ape. I mean once you kill something, the corpse is your rock for your ranged attack next turn. Can't get much better than that.

Im a fan of chickens. Turn things into chickens, put them in your bag of holding, then they dead.
