Last session my players encountered a noble Djinni. The Djinni ofered the players a Wish. Guess what? Instead wishing something related to the story the players wished something for themselves, and not only that, the bard convinced everyone to wish a Horn for himself.
What he wanted: A Horn that gives inspiration to everyone and makes all the enemies to flee.
Here's what I tought:
- To use the following benefits you must be a Bard. Requires tuning
- Frighten 1/day: All enemies within 30ft radius must take a wisdom saving throw or will be frightened for 1 minute. The DC is based on the bard spell DC
There’s the magic items, Instrument of the Bards. None of them are horns, or do what you are asking for, but you could maybe re-skin one of them. And get a sense of how strong this would be. Otherwise You could homebrew it.
That seems fine, if not quite what they asked for.
I'd probably do it slightly differently.
Use once a day. All friendly creatures within 60 (or more) feet get a bardic inspiration die. All other creatures in that radius make a wisdom save or get frightened of the wielder. Both the die and the save ought to be fixed values, since they are properties of the item.
Is it balanced? Probably not, though frightened isn't that amazing, but it's not going to break anything, and they burned a wish to get it, so let them have fun. More importantly, this thing is:
Loud. The sound will carry way more than the affected radius, alerting everyone nearby.
Dramatic. This is an Item of Note (pun not intended). The bard and their companions are going to get a rep, with all that entails.

If the party is a high enough level where things like wishes are actually a thing (Tier-3+ I imagine), then I would think bigger if it were me. I would name it the “Horn of [PC’s Name],” make it Legendary, base it off of a wand of fear (but obviously change it to a horn), change it to use the characters bard spell save DC instead of a fixed DC, and add the following property to it:
Aria of Inspiration. As an action you can play this horn and expend two of its charges and a single use of your Bardic Inspiration to inspire multiple creatures. Each creature of your choice within 30 feet who can hear this horn other than yourself gains one bardic inspiration die, matching the size of your bardic inspiration dice.
That aught to be pretty close to exactly what they wanted.

Yeah it's a good idea too. I prefer the one above, but yours is more fit for their level.

One other thing. It's better to make the enemies to Flee or just be frightened? If they flee I will have to handle chase situations and we're playing online and they would problably left the battlemap which is bad, and so I was thinking about just make the enemies frightened of the wilder.
At 7 charges that would be 7 uses of the 1st-level spell command, but with only limited commands, or 3 uses of what is equivalent to the 3rd-level spell fear or of the Aria and one use of command. 🤔 Hmmm…. You could drop it to 3 charges, that should do it.
It's not everyday you get a wish. I would go bigger like the previous poster said.
If the bard wasn't specific enough in his wishing for the horn would you ever say "Wish granted, a large horn sprouts from the center of your head and the Djinn vanishes!"
