Are you looking for monsters as written, or are you okay with homebrewing?
A really powerful ghost would be cool. You could even give it very limited offensive capability, but make it much more difficult to kill than even the other 4. Make it hard to detect, hard to ward against, able to be invisible, unable to be killed except by [insert artifact or rare spell here], and supremely intelligent, and it becomes a super-spy.
What about a 'master zombie'? An intelligent being that can inhabit (and strengthen) any dead body. Kill the body, it just takes over another one. Another one where finding out how to kill it is more of a challenge than the damage it does (although it can take over dead war elephants, dead apes, dead demons, etc). But it can still wreck your festivities until you do kill it. This character could also play the Pestilence role--give him some magical abilities to spread disease or parasites on contact (or via spell). Call it The Rotting Lord, or something like that.
You don't have a vampire on that list either. It's a more obvious choice, but a master vampire is a classic choice.
You might also think about putting in a less intelligent, brute type on there. A suped-up wight, for example. Clearly not the leader of the group, but the one Orcus drops in when it wants to just outright kill an entire city. Maybe the others don't even have great control over it--the Hulk of the group.
An undead elf of some kind could be cool, depending on your world or party. Elves are so gosh darn good and high and mighty that one of them becoming a master of the undead and serving Orcus could really piss some people off :) Maybe the Death Knight is an ancient elvish warrior who was a champion of good, but turned to undeath when he died in order to save his wife from something something, and has now turned completely away from the good he used to serve all those centuries ago.
There's no reason you can't boost your favorite iconic undead creatures or reskin the lich to round out your dream team.

Also, give some strong consideration to Brotherbok's suggestion of diversifying the races represented in your team. You can have a lot of fun with this.

I want a team worthy of being Orcus' henchmen. I want a five creature team, but am stuck at three.
Lich and demilich are pretty different. And I recommend a Mind Flayer Lich (Illithilich). The Skull lord, vampire spellcaster, and death tyrant are also all good picks.
