The general idea is a survivalesque encounter, where a single player must deal as much damage as quickly as possible. The worm has relatively low AC and the distance to the lair is not absolute, so there is sufficient room to "negotiate" this potential deadly encounter. A regular adventurer with a swim speed of 15 feet and decent Constitution (say +1) can swim 300 feet before it starts suffocating, which should be enough time to get back to the surface. If a players attempts to continously escape the grapple and not understand it has to fight, it could go a lot worse and the worm will keep trying to drag it down.
The goal I'm going for is a combination of instant panic, screams and a possibility of nightmarish sleep after the session.
