It's kind of interesting when you get in an encounter where both sides are aware of each other, but neither has decided to resort to violence yet. Finally someone decides to "draw".
I think RAW is that you just roll initiative. The character who made the first move might not be the first one to act. Which makes sense. You see them going for their weapon or spell focus, and if you can react fast, you can hit them first.
But maybe just giving the person who "takes the initiative" advantage on the initiative roll is realistic, and also could encourage moving the plot forward. I would also potentially give advantage to any character that has drawn their weapon or has their hand on the hilt, but that action might signal hostile intent to the other party and raise tensions, so you'd want to use it when appropriate.
