Szazzul growls from within his bubble, "My swamp will rise against you!" Swarms of biting and stinging insects crawl from the ground and take to the air, obscuring vision and attacking everyone within the ring (20' diameter of the 30' ring). Everyone make a CON save, DC 15, or take 10 damage (4 on save). If you end your turn in the cloud, it deals an extra 6 damage (2 if you succeeded your initial save).
Prisoners' CON: 20, 8, 10, 8, 3, 4, 13, 20, 19, 11, 9, 17. The gaggle of beaten and unarmed prisoners are no match for the swarm and two of them fall within seconds as the others scramble between the horde and the swarm.

Does it specify anything such as once per round or as a bonus/reaction? If it just says "when you deal damage, then go ahead and add it.

Re-reading through the exchange so far. Rex appears to be the only one unaffected by Szazzul's insects since he dropped in and ran for melee. Since the combat area is 30' in diameter and the insects are 20', there is a 10' gap between the swarm and enemies.
