How would anything be a surprise attack? Can you point me to this mechanic? A coordinated group of sea dwelling amphibians jumps onto your boat. You are not surprised, but how would they be surprised? Or is that just the confusing name of a feature?
I also try to mention this often, as part of my initiative system even if you win initiative you can just move your turn to go after the monsters, so yoi can use yoir whole turn instead of just reaction to use one of BA, A, (magic action), or movement. But if yiu really want it to be the triggered reaction, that's totally fine. I just felt bad using yiur whole turn to take out 1 of 9 random fish men.
Realizing that the above is given as a reaction but the only person to post for 2 days while we wait on 2 more, I wanted to do something without jumping the gun and having monsters that will be dead before their turn begins attack PCs, so they did attack before Sparhawk, but after Tatsat and Dallas.
<I think this has identified an issue with me rolling initiative for everyone: I already said they go before the monsters, which makes it awkward for me to push them down the initiative list, so I'm more inclined to wait for them before doing that than I would be if they just had not rolled initiative yet.>
<I'm also really excited for MM '24 to drop so I can see official fixed stat monster initiatives. Mine are based on my own formula which might vary quite a bit from what the designers have come up with.>

It seems pretty clear.
need to surprise your opponent and you need to hit then you get bonus damage… and this can only be done once
what specifically is the question?

The assisting Fishman grabs the netted Baron and hauls him into the water
Two more pairs of kuo-toa wmploy similar tactics against Caotian Sparhawk and Dallas Hullbreaker
