Ooc: I do have a question about the rolls after an attack roll exceeds AC. Why are we doing them? When an attack roll meets or exceeds the targets AC, that's a successful hit. There shouldn't need to be any further rolls after that, unless there's an additional condition inflicted by the attack.
If there's a house rule that we're using, that's totally cool. I'm not try to criticize, just understand.

Then roll initiative (roll 1 d20 without any modifiers). By default I will have last initiative.
Rules:

If it comes out to be more than the AC(armour class) of the defender , the hit dice is roles to determine damage. Saving throw is made. Bonus action is performed. All effects are taken into account and the total date inflicted. If the attack misses and you are in the attackers range, he or she may perform a bonus action then only. If anyone is engaged in melee combat with another character, any other attacker will have an advantage and bonus action for the defender will not be applicable.
Dice rolling: whenever you roll a dice, write before it which dice it is and do not add the ability modifier. Write all modifiers separately. Also mention advantage or disadvantage and what the dice roll is for. For example:
Trial dice roll ,1d20, advantage, modi=+2 : 4 ; 12
Here the dice roll will be taken and the modifier will be added manually. So the result will be (whatever the greater roll is)+2. If a 1 is rolled (in case of d20) it is an instant miss while a 20 will be an instant success even if the dc of a task is more. * During combat, a 20 will also add a 5 bonus of its own.
*Homebrew to increase the challenge and fun of the game.
Others: daily ration consumption is the official one. Some creature can be consumed and there fill will be given beforehand while some creatures will be unknown and hence consuming them may fill you or have other effects(deadly or good). Stealing from each other is allowed. To whisper to each other use private chat(for hidden conversations or planning treason). All other rules are official rules.
I believe that no one will try to meddle with dice results or improve stats without reason(for a fair game).
The oncoming story will also include puzzles and hard choices which must be taken.
Everyone has full rations, water, health etc. All equipments are now unused.

Then party stands at the edge of the deep chasm. They are washed in golden light as there torch light reflects off the roof. Gold nuggets, the size of fists are embedded deep in the roof above the chasm. That much gold must have been worth a hundred palaces. The chasm is very deep and the smell is very stale. Sol detects the presence of undead a from the chasm. The difficulty level of the chasm is 20. If you have a rope and want to cross, roll dice under an advantage. So what will you do?

Quin looks over the edge and his eyes begin to sparkle at the site of all the gold. Eager to get at the gold but wary of the gap he moves along the chasm in hopes of finding either another way across or a better anchor point to start from. As he scans the ground along the edge he pauses his search to uncoil his rope and unpack some pitons in preparation for a decent should it come to that.
Perception: 12 +2
If no other way down/across can be found them Quin will set pitons and decend.
Across the chasm, the tunnel continues ahead. There is some soft rock on the other side of the chasm, perfect for attaching pitons. A large gold nugget on the top has a hole in it, wide enough for a rope. Sol detects the presence of undead from the chasm. The other side also has stalagmites protruding from the roof. On the walls some symbols are engraved.
