Of course, everyone thinks of adventuring to different planes. From the Feywilds to the Abyss. But, I like to stray from the lines a bit. So, here's how the Shadowfell came to be in my campaign. Players: my players, exactly. Do Not Read for Plot Reasons.
Once upon a time, a child imagined up a world. a world of piece, and happiness. where he could fill his dreams up to the brim and fill in the blanks here. Of course, the child grew bored. he wished to have a contest, one of strength. The Arena of Beasts. However, the kid, who's form was split into 5.5 pieces building this, knew of a great evil in hi home. A door, that only appeared to who it wanted. One Day, a dragonborn with evil in it's heart, name Balzavur, opened it, and he was consumed by the evil and drawn inside. He hasn't been seen since. This... isn't the villain.
No, the villain is the simulacrum used so he didn't die in the arena, which contained a piece of his soul. He grew wicked, and eventually flew off to a different plane. The Shadowfell. Though this had been long created. No, he wanted to awaken the forces that made this plane. The Hideous Titans.
Epee Forte, a scarred and burned titan, unkillable by war, although fueled by it. He could harness energies of shadow and light into his slices with his halberd. Creator of Hatred and destruction, obviously the most powerful.
Bruenor Megashield, the invincible, able to survive anything. He commanded bolts of heaven to guard him and smite others. Of course, he abused this power. He brought the darkness to this plane.
Ohio James, The shifter. a human who could change into any beast, and wielded a magic-infused club. He brought the wilds to this plane, the least powerful.
Rec Miring, a member of the crime ring gang. He held a heritage of demons, cheating to win. He brought the undead to this land.
Bill Rebuke, the innocent. Whatever these men did, he barely did it. once. He is the 3rd powerful, however forced into bidding by Epee and Bruenor.
These 5 participated in an arena fueled by the Deities of Conquest. They had cheated, and deserved punishment, and were sealed in a long forgotten plane. They built it up, and when it threatened the Material Plane, they sealed them in a bubble of darkness. Balzavur's simulacrum, who has mastered the ability of both Bruenor, Rec, and Ohio, in the ability to change into a Giant dragon of Shadow, seeks to release these creatures into the material plane one again. Until he, by accident, brought 6 people into this plane....

I wanted to simplify the planes of my homebrewed reality, and so condensed a lot.
The Abyss and Nine Hells are the same plane. There is no Blood War here. The Nine Hells serve as a capital, or a single bastion of civilization among demonic chaos. The demon lords remain near equal to the arch devils, but the two are equally focused on the inner planes.
The Celestial plane serves as its counterpart, and both planes are the Divine outer layer.
Internally, there is the plane that reflects the material world, combining both the Feywild and Shadowfell. The common denizens of both are spread across the entire plane, though there are obvious domains named for the original planes. The Unseelie Court is within the Shadowfell, for example.
The Elemental Plane serves as the glue between planes. Elemental chaos is everywhere, but the closer you get to the material plane, the more ordered and stabilized it becomes. Each plane of existence is really just the elemental plane having become stabilized enough, with enough mixture of elements, to form a separate, divided domain.
How this affects the lore of my setting is that I've refocused the Blood War to be Fiend vs Fey, as the Feywilds and Shadowfell, by circumstance only, block the path to the material plane. However, a cult worshipping a particular fiend is attempting to circumvent this issue by boring a hole through the elemental plane. Effectively giving their fiend-god a direct path from its hellish domain to the material world.
What is or is not a god is a relative term here. The celestial plane technically holds the only true gods, the most powerful entities. But they keep themselves separate by design. For fear of breaking the balance, they do not depart their plane. The most powerful of fiends have no such compunction, and so their influence upon the material world is equally god-like. Similarly, the Archfey and Primordials, by sheer proximity to the material world, are better able to affect and influence it. Though far weaker in true comparison, what the mortals experience of each is comparable in power.
