If I readied an action against someone that was going to cast disintegrate, and used my action to cast resilient sphere, would that work in time to save the target of disintegrate?
"When the trigger occurs, you can either take your reaction right after the trigger finishes or ignore the trigger. Remember that you can take only one reaction per round."
So if the trigger you choose is, "the enemy casts disintegrate" (leaving aside the question of how you recognize that specific spell being cast) then you will take your reaction right after they do so, and it will be too late.
If you choose the trigger to be "the enemy starts casting a spell" then your spell would trigger first, but you won't know what spell the enemy's casting or who the target is.
Your DM might allow an arcana check to identify the spell as the enemy starts to cast it but I don't see anyway to justify figuring out the target until it's too late (assuming multiple possible targets).
I've always used passive Arcana to be able to determine if the player can recognise the spell being cast. For the example of Disintegrate, the player would need to have a passive of 16 or higher (DC = 10+Spell level) and need Disintegrate on their spell list. If they meet that requirement, then they know when the spell is being cast.
Yes, like the spell "Dispel Magic", a spell DC is 10+spell level (which is not the same as the DC of the character). Arcana check against that DC and your charcter will know which spell is beeing cast.
Passives for all the different skills checks can be used in different ways. Just because Perception is the most common passive, doesn't mean it's the only one.
I'll use a PCs Passive Insight when I have an NPC being Deceptive, however if the PC states they wish to sue their Insight, then I let them roll. This way I can help further the storyline without needing to be bogged down in rolls. Same for Investigation - if a PC just states they are searching the room/bodies I might just use the passive to speed thing up. That all being said, it means having the passives of every character listed down (I use Roll20, so can easily access a player's sheet without them knowing, and can make handouts for myself with the info)
I was going to say the same thing as Sorce pretty much! It wouldn't be possible in RAW but its too awesome a moment not to house rule in on the fly. I would probably have then roll rather than use passive. Its probably not a situation that would come up regularly so adding that level of tension might make it cooler when its pulled off!
