So one benefit of trying the new Encounter Tracker is that I learned I am an idiot. I have been tracking initiative all wrong through the course of an entire 5E campaign (Strahd) and several other stand-alone sessions. I also taught 2 upcoming DM's this incorrect way of doing things.
I think I was used to the way things used to be in previous editions (I'm 50) so I was having players roll initiative at the start of every round. When I was examining the Encounter Tracker I was wondering how to update the initiative and realized that you should only roll at the start of the encounter and not the start of each round. So many pieces of paper wasted tracking that round by round. D'oh!
What's the biggest rule gaff that you made that entrenched itself into your campaign?
Me stacking bonus effects and misunderstanding "resistance" is not "immunity".
That's what I get from learning D&D by watching new players streaming their first campaign.🤦🏻♂️

I am 99.7603% convinced that the digital dice are messing with me. I roll high when nobody's looking and low when anyone else can see.🎲 “It's a bit early to be thinking about an epitaph. No?” will be my epitaph.
I haven't made that many rules mistakes, but my biggest goof was thinking a potion of healing only cost 25gp and arguing with the DM about it (partly in character).
I was granted my price in exchange for buying in bulk, and after realizing my mistake, I apologized next week.
I think for me it was misunderstanding how Thunderwave worked... thinking that it essentially created a burst around my character... instead of just firing in an area in front of me, and I almost killed my pet goat, but luckily she was juuuuust out of range.

I like rolling init. at the start of every round. :/ although it is slower..
My party thought that the Paladin Smite Spells added their damage to every attack for the whole minute.
